New Multisociety Guideline Uses Five Acute PE Categories to Guide Care

A new joint ACC/AHA/peer-society guideline for evaluating and managing acute pulmonary embolism in adults introduces five Acute PE Clinical Categories (A–E) to stratify severity and guide therapy across emergency, inpatient, and post-acute settings. It recommends low-molecular-weight heparin over unfractionated heparin for initial anticoagulation, favors direct oral anticoagulants over vitamin K antagonists when appropriate, and supports continuing anticoagulation beyond 3–6 months for selected patients; it also emphasizes PE response teams (PERTs) and notes future evidence gaps.

2026 Acute PE Guideline Introduces Five-Category Risk System to Guide Management

The 2026 AHA/ACC guideline for acute PE introduces a five-category Acute PE Clinical Categories system (A–E) to sharpen risk stratification and guide therapy; patients with higher risk (categories C–E) or cardiopulmonary failure should be hospitalized, and for those eligible for oral anticoagulation, direct oral anticoagulants are preferred over vitamin K antagonists to prevent recurrent VTE and reduce major bleeding.

High Prevalence of Recreational Drug Use in Cardiac Intensive Care Patients

A study conducted in France found that many recreational drug users who suffer acute cardiovascular events end up in cardiac intensive care units (ICUs) without disclosing their drug use. The study revealed that 11% of patients admitted for acute cardiovascular events had a positive drug screen, with cannabis being the most commonly used drug. Patients with positive drug screens had a higher risk of in-hospital major adverse events. The findings suggest the potential value of routine urine screening in selected patients with acute cardiovascular events to improve risk stratification in intensive cardiac care units. However, further research is needed to assess the prevalence of recreational drug use and its impact on clinical outcomes.

Early Pregnancy Blood Pressure Patterns Can Predict Hypertensive Disorders

Certain blood pressure patterns in early pregnancy, when used in conjunction with other risk factors, can accurately predict and stratify the risk of hypertensive disorders of pregnancy, such as preeclampsia and gestational hypertension, regardless of race or ethnicity. Researchers analyzed data from approximately 250,000 pregnant patients and developed six distinct blood pressure trajectories that demonstrated excellent discrimination in identifying patients at risk. Implementing a risk stratification program based on these findings could significantly reduce adverse outcomes for both pregnant individuals and infants. The study highlights the potential for precision care without expensive assessments and could influence practice standards for treatment.
