"Boeing's Safety Culture Under Scrutiny: FAA and Congress Find Flaws"

The study also highlighted issues with Boeing's SMS procedures, restructuring of the ODA unit, and lack of human factors consideration in aviation safety. Boeing has responded by acknowledging the need for further improvement and has been urged by the engineers and technicians union to implement an Aviation Safety Action Program to address safety concerns without fear of retaliation.
