A new study challenges the traditional belief that relapses are the main driver of disability worsening in early relapsing forms of multiple sclerosis (MS). The study found that most disability worsening events occur independent of relapse activity across all types of MS, including clinically isolated syndrome (CIS) and early relapsing-remitting MS (RRMS). This suggests that progression without relapses, known as progression independent of relapse activity (PIRA), is not exclusive to progressive forms of the disease. The findings highlight the need for a standardized definition of PIRA and further research to better understand and measure disability in MS.

A survey study of over 700 people in the US found that exposure to stress as children or adults makes it more likely that people with multiple sclerosis (MS) will experience more disability and/or a heavier burden from relapses. Childhood and adulthood stressors contributed to the level of MS disability, while only adulthood stressors were found to contribute to relapse burden. The study also found that birth seasonality impacted disability, with participants born in the spring having 39% higher odds of disability compared with those born in the summer and winter. More research is needed to fully investigate the impact of stress among people living with MS.
