A 27-year-old man from Tunisia suffered a penile fracture in two places after rolling onto his erect penis while sleeping, resulting in intense pain and swelling, as well as an "eggplant" deformity. Despite waiting 36 hours before seeking medical attention, he underwent emergency surgery to repair the fractures and experienced no lasting issues with urination or sexual function. This unusual case highlights the potential risks of penile injuries, which are typically associated with vigorous sexual activity, with the "reverse cowgirl" position being the riskiest.
