An Oregon hospital is under criminal investigation after reports emerged that up to 10 patients died from infections caused by tap water injections, which were used as a substitute for fentanyl. A nurse is alleged to have replaced the pain medication with tap water to conceal the misuse of the hospital's drug supply. The resulting infections, including pseudomonas, proved fatal for patients in the ICU. No charges have been filed yet, but law enforcement is actively investigating the case, and the hospital is cooperating with authorities.
