Gurnee pizza manufacturer cited by OSHA for fatal machine accident.

Miracapo Pizza Company, also known as Little Lady Foods, has been fined over $2.8 million by OSHA for safety failures and lack of training that led to the death of a 29-year-old worker in a machine accident. The worker was using compressed air to clean a spiral conveyor when her head became caught in the machinery. OSHA found that temporary workers had not been trained or given the authority to stop equipment from moving before cleaning. Miracapo was cited for 16 willful egregious violations, one willful violation, and 12 serious violations, including five serious instance-by-instance violations of two standards on different machines. The company has been placed in OSHA's "Severe Violator Enforcement Program."

Labor Dept. Labels Dollar General as 'Severe Violator'

Dollar General has been added to the Labor Department's Severe Violator Enforcement Program due to 111 instances of workplace safety violations found in over 270 stores since January 2017, resulting in over $15.5 million in penalties. The violations include obstructed fire exits and aisles cluttered with boxes of merchandise. The company has contested many of the penalties and is now in early settlement discussions with OSHA. Dollar General's business model relies on lightly staffed and relatively small box stores with high sales volume, which creates hazards for employees and customers.
