
Faulty signal connections during repair lead to deadly India rail crash
An official probe into India's worst rail disaster in two decades has found that faulty connections made during repair work on a rail-road barrier caused the deadly crash. The investigation revealed that modifications to the signalling circuit, made to fix frequent problems at a nearby barrier, led to a faulty connection in the system. This resulted in a passenger train colliding with a stationary freight train and then hitting another passenger train. The report highlighted lapses in the signal and telecom department and a failure to follow standard operating procedures during the repair work.
